Textbook Purchasing Forms

1. Textbook Quote Request Form
   • Required for the Textbook Depository
     • Textbook depository can fulfill quote requests for different publishers
     • Only adopted textbooks can be purchased with allocated EEF dollars

3. Textbook Order Form
   • Required to place an order
   • Signature is required to complete the order

Order packets must include all three completed forms to be processed. May 1st is the final deadline to order textbooks. Any orders received after the May 1st deadline will not be processed.
